This facility is for the purpose of confidentially sharing information with the college surrounding your (possible) exposure to COVID-19 and the steps that you have taken.

Please note the following in regards to the voluntary sharing of your information

  • All records are stored securely and kept confidentially.
  • For security, no information is transmitted via email (please don’t email any certificates).
  • All records are utilised solely for the purpose of record-keeping.
  • Ongoing access to this information is restricted to the college director.
